The History Channel had a two-hour feature on 'Extreme Airports' around the world. They mentioned: San Diego (KSAN) Vail, CO (KEGE) Madiera Courcheval Gibralter St Maarten (Princess Juliana) St Barts TNT or Toncontin at Tegucigelpa, Honduras Lukla in Nepal

Most of us on the original Forum beat Courcheval to death. However, most of what made it so hard for us was the garbage that FS generates as obstacles that are not there in reality. Reshaping a mountain is tough. But cutting down trees growing too close to a runway is somethng that is done all the time. The video showed no obstacles except the big mountain at one end that forces approaches only to runway 10.
Gibralter's difficulty has nothing to do with terrain but with politics. You must make sharp turns to avoid being "captured" by Sanish airspace.
Traffic and the parking garage near the end of the runway are the only things making KSAN difficult.
Weather is the main problem at Vail.
I did not see a problem at Princess Juliana nor have i had any trouble in many FS landings there.
St Barts is a little challenging at 2100 ft
They didn't know anything about Saba. At half the size of St Barts and stuck next to the side of the mountain, Saba makes St Barts look like child's play.
Some of these might be worth a look if you have run out of all other interesting things to do in your airplanes.
