Reporting is almost, but not as, bad as the program called "Air Disasters", or something like that. It rolled. But the idea of "flipping, hitting, pushing", and "switches" is a bit off center. Rudder trim doesn't work that way. At least on 737's. Door locks were originally switches, but now the lock can be a switch or a very small rotary knob. Depends on whatever. However, making the wrong move does happen more often than most people think. Mistaking rudder trim control for a door lock control does show a problem in paying attention. Question is "why." Fatigue? Current bad situation between pilots-management? Personal problems? etc, etc, etc. I was flying a Twin Otter, IFR, in IMC at about 6000msl. Winter. Up ahead you could see huge cu's. As we got to a point about 5 minutes from the clouds, I reached up, looked at the pitot heat switch and flipped it ON. About 2 minutes later, the co-pilot reached up and I heard him flip a switch. Thing is, I remembered that all the switches were in the correct position when I turned on the pitot heat. So, I looked up at the overhead panel, and saw that everything was in the correct position. "I wonder what he did?" I thought. So, I asked him. His answer- "I turned on the pitot heat." "No, I did," I responded. "No, it was off- I turned it on." We both studied the panel- everything was okay. Somebody did something because we both hit a switch. "Well. We're still flying, so I guess we're okay," I said. What happened, or, who did what Later, I went to a company flying B99's (fun airplane- especially compared to the Otter). On a rare occassion, a co-pilot would turn off the radio master by mistake just as we entered the flare, as it was right next to the, mmmm, don't remember- maybe the beacon aural switch, or some other switch. It goes and goes even in the big ones. lou.
